| ANC (cells/mm³) | Classification | NCI Risk |
|---|---|---|
| < 500 | Severe Neutropenia | Grade 4 |
| 500 – 999 | Moderate Neutropenia | Grade 3 |
| 1,000 – 1,499 | Mild Neutropenia | Grade 2 |
| 1,500 – 7,700 | Normal Range | Grade 0 – 1 |
| > 7,700 | Neutrophilia | Grade 0 |
1
Neutropenic Fever: Defined as a single oral temperature of 38.3ºC (101ºF) or >38.0ºC (100.4ºF) sustained for >1 hour in a patient with neutropenia. Prompt initiation of empiric broad-spectrum antibiotics is imperative.
2
Dynamic Measurement: ANC is not static. It is often measured daily in critically ill patients to assess the bone marrow’s response after chemotherapy (expect drops during nadir).
3
Risk Stratification: Neutropenic fever usually results from direct toxic effects of chemo on mucosal surfaces. Consider utilizing CISNE or MASCC scores for further risk stratification.
