SOFA-2 Score

Sequential Organ Failure Assessment (2024 Update). Used to track organ dysfunction and mortality risk in ICU patients.

Brain GCS / Motor / Delirium Assessment
Respiratory PaO₂:FiO₂ (or SpO₂:FiO₂ Ratio)
Cardiovascular MAP & Vasopressors (μg/kg/min)
Liver Total Bilirubin (mg/dL)
Kidney Creatinine / Urine Output / RRT
Hemostasis Platelets (×10³/μL)

SOFA-2 Score: 0

Higher scores correlate with increased mortality risk.

Respiratory: Scores 3-4 require both the PaO₂ ratio AND advanced support (HFNC, BiPAP, MV).
Cardiovascular: Dose calculation is based on the sum of Norepinephrine and Epinephrine base.
Renal: RRT (Dialysis) usage automatically assigns 4 points regardless of creatinine levels.
CNS: If full GCS is unattainable (e.g., intubated), use best motor response criteria.
Reference: SOFA-2 Score (2024 Update). Based on Sepsis-3 Criteria guidelines.
